Per evitare collasso aumentare numero delle borse regionali a 200
Accade in tutta Italia, ma la Sicilia è tra le regioni più in difficoltà, con Lombardia e Lazio: nei prossimi dieci anni - secondo il report dell'ente previdenziale dei medici Enpam - ne andranno in pensione nell'Isola 2.897 su 4.080. Più di sette medici di base su dieci. E non ci saranno abbastanza giovani per coprire i buchi. Ogni anno la Sicilia forma 120 nuovi medici di famiglia attraverso il corso di formazione triennale di Medicina generale. Conti alla mano, fra dieci anni avremo 1.200 nuove leve e ne mancheranno più di 1.300, se si computano anche le stabilizzazioni di camici bianchi già in graduatoria. A lanciare l'allarme è stato per primo il segretario regionale della FIMMG Sicilia, Luigi Galvano. L'ultimo appello all'assessorato alla Salute risale a gennaio, ma è rimasto finora lettera morta. «Per evitare il collasso - dice Galvano - abbiamo chiesto di aumentare il numero di borse di formazione regionali fino a 200 all'anno. Già quest'anno sono aumentate da 80 a 120, ma non basta». La Regione mette sul tappeto 2,5 milioni di euro di risorse annue per il corso triennale al quale si accede tramite una selezione. Nell'ottobre scorso hanno fatto domanda ben 1.500 aspiranti medici disposti a essere pagati appena 800 euro per tre anni pur di conquistare l'attestato, a fronte dei 1.700 euro di un medico specializzando in corsia. Segno che diventare medico di famiglia fa ancora gola ai giovani laureati.
Dopo il corso, si presenta domanda per essere inseriti nelle graduatorie di continuità assistenziale e di medicina di base delle aziende sanitarie locali, che vengono periodicamente aggiornate, e partecipare ai bandi dei posti vacanti che vengono pubblicati due volte all'anno, per diventare titolari. A marzo, a Palermo, saranno 18 i posti disponibili. Nel frattempo ci si può arrangiare con sostituzioni in guardie mediche e studi medici che fruttano fino a 2.500 euro lordi al mese. Una volta diventati titolari, è come firmare un'assicurazione sulla vita: «Un "massimalista" che raggiunge il tetto di 1.500 assistiti - spiega Galvano - guadagna seimila euro lordi al mese. Ma bisogna decurtare il 33 per cento di tasse, le spese per la segretaria, le utenze, l'affitto. Al netto restano in tasca tremila euro». Lo stesso stipendio di un medico ospedaliero che lavora il doppio (40 ore settimanali contro le 20 ore minime di un "massimalista") e corre più rischi legati a errori e denunce. Ma allora, perché questi venti di crisi? «Negli anni Ottanta e Novanta - ricorda Galvano - prima dell'obbligo del corso di Medicina generale, era possibile diventare medici di base anche senza essere specializzati. Ci fu una grande infornata di medici nati tra la fine degli anni Cinquanta e Sessanta che oggi hanno maturato l'età per andare in pensione».
